In the All Fields section of the Property Pane I have arranged several  fields as sub categories. I can see they are subordinate when I activate Tree View. How can I recreate or enable this structure with my forms? In other words I would what was created in All Fields to be seen in Forms. It seems like this should be possible.

Forms do not show fields in hierarchy. This is by design.
However in forms, you can choose the order in which fields are listed

Why? Or better, why not allow for the user to create a hierarchy to suit the particular form design? This way a user could organize several forms differently to reflect differently needs.

This is precisely how forms work. You can create as many forms as you wish, select which fields are shown and in which order. Forms can even automatically assigns values to fields. Forms can be used to apply a series of values to item(s) in a single click.
 
Unless I don't understand what you mean, it seems to me that forms showing a hierarchy of fields would be an unnecessary complication.

I understand what you have described and this is how I currently use forms. For organizational purposes it could be helpful to have groups of fields under a heading. For example, I want to track assignments given to ten staff. My form is long and the ability to isolate "staff" and then to expand the names below would be helpful. I have to do it this way because as far as I know IQ will not allow multiple selections in drop down lists (unfortunate). While I am writing this, I thought of a inelegant work around, but what I described above would be better for me.

I would split the form in smaller forms, and you can open/close these as required.

I was just laughing to myself. This was the work-around that occurred to me as I was posting.
