I didn't use this for awhile and I don't usually use it very often, but...
I thought I understood that when the item doesn't belong to the grid ( and that means that in property pane the grid field won't be checked) it won't show when hierarchy is off. In my pre-rel27 it's not the case. More so, when I check the grid source field for that item it jumps up to the top level.

If it's a child of an item which is in the grid, it should show up in the grid (when you expand the parent item) unless you have specific filters to hide this child item.
 
As for the item jumping to the top level, this is a known issue, which will be fixed in v0.9.27

But it used to be different -- I remember that that's why I assigned the rule to some grids that all the items will belong to the grid (the grid field checked in the property pane).
 
Anyway, in my grid when I turn off the hierarchy nothing happens -- and as I understand i should get the "flat" view.

Jay, I know this is a bit old, and some of these features (hiearchy ON/OFF, etc.) might somewhat change in the future.
 
But the principle is simple : "hierarchy OFF" only puts all items that meet the source and filters at the top level. If your items don't meet the source and filters, they won't show as TLIs. As simple as that.
 
So you should check if items that don't show as TLI actually meet the source/filters.
