I opened the standard file I use for templates and found that many of the toolbar icons were missing.  I don't understand how that happened.  I thought I understood toolbar settings are determined by the last file closed.  If I have all files set up with the same toolbar layouts and don't make any changes, then all files (including new ones) should keep that layout.
 
I don't know why it lost the toolbar layout but since it was just one file, I decided to re-create the layout manually.  I pulled up the "all commands" list and found that the tool icons have apparently been changed.
 
1) There's no longer an icon for "save file" (only "save as").  If I drag the "save" tool to my header it inserts a labeled icon ("Save") which takes up too much space.
 
2) There's no longer a command to re-number items.
 
3) There's no commands to sort items.
 
I stopped at that point.  What's happened to all the tool commands and icons?  Most commands have no associated  icons, and many commands are missing altogether.
 
Update: If I open a current file and check available tool icons, they're there (didn't do a systematic check).  Why are the missing on the older file?

Ugh, spent two plus hours trying to figure this out, reading and re-reading the help file, and finally somehow stumbled on the answer.
 
If you have only the home tab open, then the menu layout is radically reduced, and all the corresponding tool icons are removed (neither are they available in the customization dialogue).  As soon as I opened a regular grid, everything was restored to the way it should be.
 
No idea what the purpose of this is, but it'd be nice to have some kind of warning in the help file.

Confirmed that hidden commands did not show in the commands tabs of the Customize dialog

Thanks, Pierre.  Ignore my tone.  I was frustrated with myself at the moment.  Since you say you're fixing it, I presume you confirmed the  commands did NOT show in the customize dialog.

No problem for the tone, it is understandable ! For the record, that bug has always been there... for years that is 
 
Nobody noticed it before, most likely because grids is the main UI. Also, finer control of the menu items is now done, based on the current view, so non-relavant commands are hidden more than before.

 I actually noticed it but thought it was "by design".  And since I'm usually working in grids, I never bothered... : )
