This is by design. Start date and item duration define the calendar item. If you want, you can define an End Date (=start date + item duration)

You can rename any fields, but it is field ID 50 that is used for appointments. See the field management dialog. Also, when you create an appointment, check the Filled Fields section of the properties pane. You should see at least 3 values:
- FieldID 1: Item: Appointment text
- FieldID 50: Appointments: Appointment date
- FieldID 51: ItemDuration: Appointment duration in hrs
- FieldID 1: Item: Appointment text
- FieldID 50: Appointments: Appointment date
- FieldID 51: ItemDuration: Appointment duration in hrs
