Why on a Custom Menu, does the "Grids" (Ctrl+Shift+O) from the Grid Context toolbar work in this way? It works properly from the standard and the item context toolbars.

Close IQ and re-open should update that menu.
The Grids button menu is a special button, with dynamic content based on the opened file.
I would need to track the creation of custom menus and fill it. Eventually...
Note, that if you had chosen to copy another Grids button (an existing one, from a toolbar or the main menu), it would have been filled with the list of grids. You evidently used the button in the Customize dialog, and this button was not initialized.
