0.9.25W is prompting me with the following dialog box when I try to edit text in the Add Item dialog. I had to go back to V3 to get rid of it :(.

Confirmed.
This is very easy to reproduce so it shouldn't be too hard to fix.
Yes, it does prevent one from entering notes using win+n or ctrl+n.

you got there ahead of me - here's what I had written out:
 
Adobe acrobat

Win+N
all below within the new item dialogue:
  1. Focus is in Item Text
  2. Move focus to HTML pane (of the new item dialog)
  3. Move focus back to Item Text
  4. Try typing - I get this error (and unable to type)

There seems to be a bug with the regular (win + N) clipper. If you use the universal clipper, I don't think you will experience this problem. See http://www.sqlnotes.net/drupal5/index.php?q=node/2498
 
On a related topic, I am not sure of the intended differences between the two types of clippers, but I am glad that at least one works well.

Both use the same UI, but Win+N doesn't clip anything : it's just to quickly jot a note.
 
However, as you say, Win+C works fine... so if you don't select anything before pressing the keys. So here you go : temporary workaround for note taking!

Thanks for explaining. I knew about that behavior, but I thought there were other differences.

There seems to be a new problem now, I think maybe it started in 0.9.25W. As shown in my screenshot in the first post, the title is being set as the item text when when doing an Add Item. It's driving me up the wall :).
 
Can you please make than an option I can turn off - I just want a blank box. At the very least, select the title text so when I start typing its overwritten.

You mean when entering notes using win+n or ctrl+n ?
Here, there is indeed text in the "item text" field -- the text seems to be captured from the title bar of whatever window has the focus.

This would be the best option IMO. It actually used to be like that IIRC.

(Pierre : As a side note : here the "add item" dialog takes a long time to appear -- several seconds. I wonder if it could be made to appear... a bit faster ? :) )

Yes, I'm pretty sure it was blank in a previous release.

The New Item hotkey no longer retrieves the window caption. Fixed in v0.9.25W3
