To reproduce:
 
1) Create an item using the enter key (I have to press enter twice here - "Enter creates a new item" is disabled)
2) Create a sub-item using Ctrl+Enter keys - a new sub-item is created (see mouse arrow in screenshot)
but a "field" drops down from the source bar with the active cursor in it (see red arrow in screenshot)

This drop-down text area  works perfectly well - I can write my text, it gets transferred to the item's item filed when I press enter.

But it is unexpected behaviour.
Note: it does not happen when adding sub-item via item's context menu.

Yes, I have seen this from time to time too. In my case, the box appeared at the bottom of the window, not near the top as you have it. I have not seen it lately though. At the time I was not sure if it had something to do with the pop up editor or auto edit. Sorry I can't be more helpful.

it seems to be 100% reroducable here, but maybe there's some factor I'm missing out on - or it's only on some systems.
Might be to do with my settings...

 Maybe so. I tested on Win 8 machine.

 
This is related to the AutoEdit setting.
I disabled that and it's back to normal.
