 Steps:
 
1- Configure IQ so that it creates a backup every 5 min.
2- Create layout  (a)
3- After first backup (5min), change the  layout (b) and save it.
4- Before next backup, forcefully close IQ (using taskmanager) and reopen it
 
--> Layout "a" will be restored, not layout b, even if it was saved.
 
IMO IQ should determine latest save and restore the newest layout. (Otherwise it defeats the purpose of saving the layout manually to avoid exactly that situation... falling back on an older layout)

Something similar seems to be occurring with grid order.  I will test this properly when I get a chance, but basically I have noticed that even after saving grid order, if IQ crashes, it reopens the database with the previous grid order.
