Clean portable 109c version (edit: windows7)
  1. open sample file (I chose with no data)
  2. menu: Tools >Help >About >Support Info (I copied IQ version info)
  3. close about dialogs
=> many of the toolbar icons disappear.
 
If I go to customise toolbars, and change icons to large, the icons show again -- but disappear again when I close customise menu.
Restarting IQ does not restore icons...

For me the toolbar icons refreshed (flashed whilst being re-drawn) for a fraction of a second (barely perceptible) but they did not disappear.
 

Icons are still gone here after rebooting using the clean portable 'install' and sample file.
The icons show as soon as I open the customise (toolbars etc) dialog -- and disappear again when it is closed.
This on a windows 7 machine.
 
Can try the same on (win.7) laptop when I get a chance.
 
Note: I believe Paul is on a Windows 10 machine.

The icons which are visible depends on the active window (tab, pane). Perhaps you need to open / create a grid to view them.
Opening the Customize dialog shows all icons, this is normal.
 
The one exception is the Home pane which does not (by design) change the toolbars, as it is seen as an advanced, configurable, menu.
