Confirming images getting corrupted in a different database than the one used in my first report.

I'd appreciate assistance.
I'm using IQ v0.9.112 Portable.
I've been pasting many images from the clipboard into different items notes (HTML Viewer).
Today I revisited some items, and realized some of the pasted images are corrupted:
 
Verifying the HTML Tag of the images, then going to those paths show the item folder (and related images) don't exist.
Here an example for the ItemID2965 in IQ HTML Tag:
and the ItemID2965 - which doesn't exist - in the file manager:
 
In another thread "Reporting situations/bugs crashing IQ v0.9.112 Portable" I mentioned some issues, which happened when I was dealing with the DB in which the images are corrupted.
 
Is there any chance those crashes caused the image corruption?
Any other known issue that could cause the image corruption?
How to prevent that to happen?

Hi Carlos,
 
Is it possible that the item you're viewing is a copy of another item ? e.g.:
  1. Create item 2965
  2. Add content / image in the Doc pane. Image file was created and put in ItemID2965 folder
  3. Copy item (XML mode). Now has an ID of say 2966. Doc pane content is copied but references image for item 2965
  4. Delete item 2965. This also deletes the associated files, hence the image
  5. Item 2966 doc pane references image of item 2965 but has been deleted
