I haven't been through all these but a couple of comments:
 
a) In HTML Export, the save and preview buttons work on the 'Use Settings' tab.  Although they are still visible on the 'Use Template File' tab, they do not function.
b) On the first tab the 'use html style bulleted list' checkbox results in the ItemHTML not being shown.  Is this intentional?

Hmm.  If you set the dialog up like those screenshots I emailed to you an hour back, do the following:
 
a) Activate the first (left) tab.  Click save/preview and it generates the grid output file and brings up a preview.
b) Close your preview, delete the generated .htm file.  Now switch to the right hand ('Use template file') tab, click save and preview.  Does not generate HTML or present a preview.
 
This is true also for the copy to clipboard button (and I'd guess the save button too)
