This is a previously fixed bug which came back -- probably when adding the "clipping info" option in the dialog or something like that.
 
So the problem is what this threads subject says... Also,  "clipping info" adds up each time the dialog appears.
 
Steps :
 
- Select some text somewhere
- Win-C to clip it,
- Dialog appears and click on "add" to add to your DB.
- Win-N to add a note : Add Item dialog appears filled with previously clipped data.
 
[Edit : I think the same can be achieved by copying some data, and then pressing WIN-N : Add Item dialog appears filled with the copied data.]
 
Anybody can confirm ?

This is a pretty minor bug, but annoying at the same time since I always have to delete the previous content before entering the new one...
 
I'm bumping it up as it is probably not a big one to fix.

I see that the problem is that the dialog appears filled with the clipboard's content.
