Here's the old forum in .sndb forum.
 
The setup is very simple--no hierachy.
 
There may a few duplicates here and there.
 
It's rather large--after a while, I started deleting the avatars.
 
I put in all the forums except the "naming" one.
 
Everything is a MHT file, so get out your Word or Blocknote to edit.
 
I captured everything, unless there was a totally blank message. So there are a bunch of "bumps", "thanks", "edited previous post", etc, that could be deleted.
 
Technique:
 
0) a large amount of experimentation with how best to do the web captures, and whether to mix MHT and HTML.
1) brute force web capture message by message,
2) categorize by forum
3) type/ cut 'n paste "Who" names (INCREDIBLY tedious, by the way)
4) type/ cut 'n paste thread name. 
5)  mutter over and over "am I ever going to get this thing done?" .

p.s. You of course win the prize for the most extensive use of IQ's web clipping. I guess it works fine for you... any suggestions for improvements for such marathon tasks

The functionality is superb.  Other than the crashing mentioned in another post (which I wonder might have to do with MHT, by the way), there are only a few things I can come up with:
 
1) I'd occasionally miss the "Add item" button, and hit Auto-Close (the words) which I almost never want for my purposes, and which slowed things down when happened. 
 
Suggestions: make only the checkbox selectable as a toggle, not the words.  Or make the "Close button" smaller and put them side-by-side. Or best of all for me, use some kind of control to set a default for the mode that you use most.
 
2) Also please consider moving the "Save HTML as MHT" closer to the Add item.  For web capturing, I use a stylus (Wacom bamboo) to select the text, then click the "Add item button", If I want to switch between html and mht, it slows me down to move from the "html as mht" area to the Add item area.

n.b. Current Grid is enabled if the current grid has a simple source. Filters may prohibit the actual display of the item. Tooltip shows the current grid name

I wonder if we could also have something along those lines :
 
1- form selection drop down menu (last selected form would be memorized)...
2- which would... preselect a bunch of fields, which would appear in an other drop down menu (in a format similar to the properties pane) speeding up the data entry in the add item window. That dropdown meny could be (or replace) the "other Fields" drop down menu shown in your screenshot.
 
That would allow for incredible flexibility.
 
What do you think ?

Oh... and BTW (I just remembered that one) : the "New In-Box item" option in the "edit" menu should be "add item" ... same as the "add item" window title. A bit confusing otherwise.

Are you suggesting that other fields be replaced by:
forms w fields
+ drop down = other fields
 
If so, I agree on the flexibility & potential efficiency of this entry form..

[Forms (drop down menu**) ]          [Fields (drop downn Menu]
 
 
This would be very easy to use.
 
Of course, the "forms" drop down menu could also contain the "available fields" option + "GridForm" options (that last one could be automatically selected when the "current grid" option is checked.

Also [kind of a weird thread to talk about all that... but I'm just following the trend...] , a good thing would be to fix the keyboard navidgation in the add item window. Some options are not easily accessible using the keyboard only, and some keyboard shrtcuts (like alt-d) don't produce the expected result.

With the addition of my comment to Armando I think this entry form is a great improvement.  The only thing I'm left thinking about is the Due
Date field. 
 
If possible it might be preferable to see simply a "Date" Field with a drop down to select what kind of date, i.e. appointment, due, start (for grid -> calendar entry), etc.
 
Then I think you would have all your bases covered.
