Note the blue repeat of the title "What are They Thinking"
This appeared along with a child called "Research", also blue
"Research" also appeared as a sibling to "Drafts" in black
I deleted the blue "research" and the other "research" also disappeared.
 
Two Questions:
 
1. How / why do the blue items appear as I don't want them there
2. Where does deleted item go? If it is permanently deleted (despite the options offered) I would suggest either an IQ Recycle Bin  (like UltraRecall or ADM) or have the deleted items directed to Windows Recycle Bin

The "other" research disappeared because, "they" are one and only one item. To be sure, you can check the ItemID, same ID... same item
  1. Blue items are context parents, you can show/hide these
  2. Deleted items are deleted. You can open a backup, and copy/paste in XML to get your item back
 

This could also be related to hierarchy settings etc
 
First check your settings under Grid menu
Hierarchy is obviously set to "on"
Hierarchy on or off?
Full Hierarchy on or off?
 
I suspect hierarchy is turned off or full hierarchy is turned off
 
I had similar problems with a new grid lately but unfortunately didnt record it properly
I created a bunch of items, made some into sub-items but on refresh they showed twice (I cant remember did they show in blue)
Anyways I got very flustered so cant say what I did or how it changed but it did change back to "expected" hierarchy (I always use Full hierarchy but may have somehow slipped up that time)
 
I know similar stuff has been talked about before but it is very confusing when it happens
