UI quirk when creating sub-item

To reproduce:
 
1) Create an item using the enter key (I have to press enter twice here - "Enter creates a new item" is disabled)
2) Create a sub-item using Ctrl+Enter keys - a new sub-item is created (see mouse arrow in screenshot)
but a "field" drops down from the source bar with the active cursor in it (see red arrow in screenshot)

This drop-down text area  works perfectly well - I can write my text, it gets transferred to the item's item filed when I press enter.

But it is unexpected behaviour.
Note: it does not happen when adding sub-item via item's context menu.

Erratic problem and annoyance

I know that this will not be able to be addressed because it does not happen often, nor can I identify antecedent commands that lead to this. There seem to be none. However this and other erratic problems intrude upon work flow in IQ and do not contribute to the perception that IQ is stable (this last thought is not from me, but from a colleague who experienced this problem  too but on her own created-from-scratch file).
 
1. Open my file.
2. Add a few items.
3. Use CTRL+2 to open children at level 2. This does nothing. Try other CTRL+Number combinations. None are working.
4. Re-start IQ.
5. Try again. CTRL+Number works fine now.
 
This has been on 10 and 11 and possibly other versions but I did not mention it. Now that I am using IQ for actual work, these glitches become more noticeable and more annoying.

Equation Examples in the Manual

Looking at the Equations page (I was thinking of adding the example that Jan gave Pulling vs Pushing row equations)
So I had a look at the Examples on that page Field Management Dialog - Equations
 
Okay,
the first one doesnt make an ounce of sense to me - it seems to me that there's a cut/paste typo on the first line, but even apart from that, there's no indication where the "Function..." script below the screenshot should go.

So i created a field = 2_blog (This is boolean field that identifies items I've collected that I want to publish on my blog)
When i identify an item as 2_blog I want to auto assign "GTD_FoloUp"
 
If I understand push, i would have some equation in the "Item" field
If I understand pull, I would have some equation in the "2_Blog" field
 
It makes more sense to me to add a pull filter in "2_Blog" but this doesn't seem possible. I guess I should understand this by now but I don't
If it's only push, won't the "item" field eventually get overloaded with multiple equations? 
 
in either case I don't know the proper syntax.
