Every few days I notice that icons that I have deleted have been restored, creating duplicate icons that I don't need or want. 
 
When I customize the toolbars, I don't pay any attention to toolbar assignments.  I just look at the header and then delete or add icons where I want them.  Could that be causing the problem or is there something else I'm doing wrong?  I did not add these icons and I haven't turned any toolbars on or off.  I couldn't find an answer in the help file.
 
You can't see the problem in action but in the screenshot below the "favorites" icon is positioned at the lower right.  On the top row there is a duplicate favorites icon that showed up 10 minutes ago.  Next to it is another duplicate ("refresh grid") icon that showed up at the same time.    There are several more dupes that  I have deleted several times but keep re-appearing over time.  There's also often a fresh batch after I do an update.

While I'm on toolbars, I think this may be a minor bug (or quite possibly, something else I don't understand).  When I added the tool icon that reads "Display Mode", I could not remove it either by dragging it off or by using the menu at the end of the toolbar (it didn't show up in the list of buttons).  I tried everything, including adding it a second time but this only created two of the same icon that couldn't be removed.
 
The only way I could get rid of it was to reset my toolbar.

Evidently, your toolbars are being reset to the stock version. 
  1. Portable or installable flavor ?
  2. Are you always using the same Windows account ? Same PC ?
  3. Anything could be deleting your settings ?

Installable version.  Always same PC and same account.  No idea how settings could be getting changed.
 
I don't think toolbars are being re-set to the stock version or I'd see massive changes to my layout.  Instead, I'm seeing dupes popping up over and over.  I'm going to monitor it more closely and try to identify exactly what's happening (see below).  Right now, I can't say when or how it happens, I just periodically notice that old icons are showing up again.

I'm going to systematically track this for a while to try to document when and where the toolbar issue occurs.
 
Here's my cleaned up toolbar at 4:10 pm:
 
 
I exited InfoQube, re-loaded the file, and got this toolbar at 4:12 pm.  Note the icons added at the upper left.
 
 
I cleaned it up again.  This time I saved the file and also entered an item I before exited.  When I brought InfoQube up a second time, the changes were preserved.  Tentative conclusions:
 
1) InfoQube does not preserve toolbar changes unless you manually save changes or take action that causes InfoQube to initiate an autosave.
2) This does not explain the majority of added icons because most of the time I do not exit InfoQube immediately after making toolbar changes.

In this particular example, you emptied the formatting toolbar instead of just closing it. The other two are identical.
To close a toolbar, right click on it and uncheck it

Yeah, I said I don't pay any attention to toolbars.  I just move icons where I want them to display.  I'm not sure why that would cause the problem.  I do the same thing with Excel etc without running into a problem.  If the formatting toolbar has been emptied of icons, shouldn't it display as blank?  It shouldn't suddenly re-instate all the icons I moved out or deleted.  Or maybe it should.
 
I prefer not using the toolbars because the commands aren't always grouped the way I prefer.  But if it's needed to make the icons display properly, I'll have to.

The difference is that the 'Formatting' toolbar is being displayed (dont know why that would happen if not shown previously).

Actually, it is displayed, but evidently emptied. See the Add/Remove vertical button still present on the extreme left

Based on Armando's excellent suggestion in this thread, I stopped modifying toolbars.  Instead I turned ever single one of them off, and simply created my own custom toolbars.  Results:
 
1. I never have to worry about my customizations being lost, even if the standard toolbars have to be reset for some reason.
 
2. An amazingly cleaner and streamlined IQ, which it looks like you are also after, IQ tends to have major icon overload.  I also turn on text and icon for some of the commands I used the most which at least for my eyes makes them so much easier to get to quickly.  (I know you already know all this stuff)

The flavor which is tested here is the portable one. You may want to try it instead of the installable.
Also, please report back the content of Tools > Help > About > Support Info

I switched from portable to installable at your suggestion because I was having trouble with the HTML toolbars getting scrambled.  I think David's suggestion will likely work. 
 
I started implementing new custom toolbars and I think I may have discovered the source of my problems.  InfoQube does not like you to move icons from toolbar to toolbar nor create a new toolbar using icons from existing toolbars.  I created two new custom toolbars and populated it by dragging icons from existing toolbars.  As soon as I was done, all the icons in one of the new toolbars disappeared.
 
I've started over again and am populating the toolbars from the master list in the customization dialogue window.  I'm turning off all the original toolbars and will rely only on the two new toolbars I'm creating.  I'm hoping this will do it.

The problem has returned and I've again lost my custom toolbar arrangement.  As suggested by Armando (thru David), I turned off all the default toolbars and created new custom toolbars. However, just now, I noticed that:
 
1) My custom toolbars have been deleted.
2) The toolbar set up has reverted to some previous arrangement using the default toolbars.
 
This time, I at least have a clue to what happened and maybe this will lead to an explanation.
 
I run two databases and often have them open at the same time.  When I lost my custom toolbars, InfoQube changed the toolbar icons to match the other database, whose toolbars had not been modified. 
 
My understanding was that toolbars were set up independently for each database.  Please clarify if that's not correct.

That explains it all... Toolbars are not per IQBase, but per user. See InfoQube Program Install and Set-up
If you have more than one instance of IQ running, the last one you close will save the toolbar config. This is the way MS Office works too...
 
I'll add code to handle this, in the meantime, a couple of options for you:
  1. If you modify toolbars, ensure that you close that instance last
  2. Run 2 different setups (2 portable IQ folders)
  3. Run each instance with a different user name. See the InfoQubeForUser1.bat file for details
I'd go with option 1...

Fantastic!  At least now I know what's going on and can fix it.
 
I think the safest fix is to revise all database toolbars at the same time, then you don't have to worry about the closing order.
